DENTAL PLAN
We offer you registration to our professional Plus Dental Membership for the fees shown in this contract, subject to the Conditions of Client's Registration noted below. Your payment depends on the membership plan that you take, as different plans have different monthly payments.
Current membership plans are:
New Kings Road Dental Practice administers your registration and collects fees on behalf of the practice. Your monthly fee is payable to New Kings Road Dental Practice by Direct Debit or continuous card payment.
If you wish to end this contract, you may do so by giving notice in accordance with the terms of the contract. We have agreed that your entitlement to our professional dental care services will start from the date shown on this contract.

In these conditions, “the contract” means these conditions and the contract you have signed; “your dentist” means the dental practice named in the contract form.
The contract entitles you to receive the professional dental care and treatment you have agreed with your dentist as described in the Plus Dental Membership plan you have joined.
Emergency appointments are covered where clinically necessary. However, any procedures, treatments, prescriptions, laboratory work, or other clinical services required following an emergency appointment are not included within the membership and will be chargeable separately.
Your entitlement under the contract is with your dentist at New Kings Road Dental Practice.
If your dentist arranges for a colleague at the practice or another clinic operated by the practice, for example, during temporary absence, to provide dental services on his or her behalf, this will be covered by the contract.
However, if you are referred for specialist examination, laboratory work, hospital treatment, or treatment by another practitioner who provides services independently of the practice, this will not be covered by the contract, and you will be responsible for those practitioner's fees.
You must pay the monthly membership fee to New Kings Road Dental Practice according to the plan you have selected.
Any other amounts due to your dentist for treatments not included within the membership plan are payable directly to the practice.
Your liability to pay the monthly fee continues until the contract is ended in accordance with these conditions, and no refund of the fee will be allowed except in the case of administrative error or exceptional circumstances at the discretion of the practice.Failure to use the services available within the membership does not entitle the member to a refund or credit.
The monthly fee may be altered by your dentist at any time; you will be given at least ten days' notice of any increase.
Following a decrease in the monthly fee, your direct debit or card payment will be changed immediately. When you are given notice of an increase in the monthly fee, your direct debit or card payment will be changed at the end of the notice period unless, in the meanwhile you end the contract.

You may be charged a reasonable missed appointment fee should you fail to attend an appointment without adequate notice. Repeated missed appointments or failure to attend recommended examinations may result in suspension or termination of the membership at the discretion of the practice.
All membership schemes have a minimum subscription period.
You may end your contract on expiry of the minimum subscription period. You must first pay any outstanding sums due to your dentist and then give not less than a 3-month notice. Your dentist may end your contract by giving you one month's written notice expiring on the last day of a month.
The practice may also terminate the contract where a patient repeatedly fails to attend appointments, fails to comply with treatment recommendations, or behaves in a manner deemed inappropriate or abusive towards staff.
If the monthly fee is unpaid one month after it is due, your dentist may end the contract immediately by giving written notice. You will remain liable for all sums outstanding to the practice. Any treatments provided before termination will remain payable.
These conditions may be varied by your practitioner from time to time. The latest version of the conditions will always be available from the practice.
The contract is with your dentist alone, but may be transferred to another dentist registered within the practice if necessary. Membership is not transferable to another person.
Nothing in this contract prevents you and your dentist from agreeing that he or she will provide treatment outside your entitlement under the membership plan. You will be responsible for paying for such treatment directly to the practice.
